Efavirenz is prescribed to treat HIV infection by preventing reverse transcriptase from working properly. This belongs to the class of medications known as nucleoside reverse transcriptase inhibitors (NRTIs). Reverse transcriptase is a part of the human immunodeficiency virus required to infect cells and to make more viruses. Efavirenzseems to slow down the destruction of the immune system.

Recommendations for use of Efavirenz 200 mg (Sustiva)

Efavirenz 200 mg is given with other antiretroviral medications to treat patients infected with HIV-1. Efavirenz 200 mg cannot stop the progression of HIV to AIDS on its own; it must be part of a regimen of anti-HIV medications. Adding only Efavirenz 200 mg to a currently failing regimen is also insufficient.

Ensure to read and understand the Patient Information Leaflet provided by your pharmacist before you start using Efavirenz 200 mg. If there are any doubts ask your doctor or pharmacist.

Efavirenz 200 mg is taken usually once a day orally by patients. Take Efavirenz 200 mg with a full glass of water. It is best if you take your dose before bedtime to avoid potential nervous system side effects. Do not crush, chew or break the tablet.

It is recommended to take Efavirenz on an empty stomach or at least 2 hours after a meal. How much Efavirenz your stomach absorbs, can be increased by food, which can increase the risk of side effects.

Dosage and direction of taking Efavirenz 200 mg

Your dosage will be decided upon your medical condition, age, laboratory tests, your response to the medication and other medications that you are taking. Take Efavirenz exactly as it was prescribed for you.

Children are usually prescribed to take Efavirenz 200 mg once a day based on the weight.

Do not take in doses which are either larger or for longer than what is recommended by your doctor.

Do not skip any of your doses. If you skip your dose or change the doses without your doctor's approval that may cause the HIV virus to increase making the infection a lot harder to treat or worsen the side effects.

Regular usage will help to get the most from this medication. Take it always at the same time every day.

In children, the allocated dose dependent on the child's weight. In such cases where swallowing tablets may prove to be difficult, the tablet can be broken open and the fillings can be mixed into 1-2 teaspoons of soft foods (eg: yoghurt). Once mixed, the dose must be consumed within 30 minutes. In the case of babies, the tablet's filling can be mixed with infant formula and delivered via oral syringe to the baby.

Missed Dose of Efavirenz 200 mg
If you missed a dose take it as soon as you remember. If it is too close to your next dose then skip the missed dose and take the regular dose. Should never double the dose in order to make up for the missed dose.

Overdose of Efavirenz 200 mg
If overdose seek emergency medical treatment or contact the doctor immediately.

Precautions of taking Efavirenz 200 mg (Efavir 200 mg | Stocrin 200 mg)?

Efavirenz must be taken with other medicines that act against the HIV virus. If Efavirenz has been started since unable to prevent multiplication of the virus, therefore, along with this you must start another medicine you have not taken before at the same time.

You can still pass on HIV when taking this medicine, so it's important to take precautions to avoid infecting other people through sexual contact or blood transfer. This medicine is not a cure for HIV infection and you may continue to develop infections or other illnesses associated with HIV disease.

You must remain under the care of your doctor while taking Efavirenz.
Tell your doctor:

  • if you have a history of mental illness, including depression, or of substance or alcohol abuse. Tell your doctor immediately if you feel depressed, have suicidal thoughts or have strange thoughts.
  • if you have a history of convulsions (fits or seizures) or if you are being treated with anticonvulsant therapy such as carbamazepine, phenobarbital and phenytoin. If you are taking any of these medicines, your doctor may need to check the level of anticonvulsant medicine in your blood to ensure that it is not affected while taking Efavirenz. Your doctor may give you a different anticonvulsant.
  • if you have a history of liver disease, including active chronic hepatitis. Patients with chronic hepatitis B or C and treated with combination antiretroviral agents have a higher risk for severe and potentially life-threatening liver problems. Your doctor may conduct blood tests in order to check how well your liver is working or may switch you to another medicine. If you have severe liver disease, do not take Efavirenz.
  • Do not take Efavirenz, without informing to the doctor:

  • if you are allergic (hypersensitive) to efavirenz or any of the other ingredients of Sustiva. Contact your doctor or pharmacist for advice.
  • if you have severe liver disease
  • if you have heart disease
  • if you have mental illness or psychosis
  • if you have seizures
  • if you consume a large amounts of alcohol
  • if you have history of injection drug use
  • Inform the laboratory staff that you use efavirenz if you go for drug screening urine test as it may give false results because of the medication.

    Do not give this medication to children who are 3 years or younger and who weigh less than 10kgs.

    If you are having surgery prior to your surgery, inform your doctor or the dentist of all the products (prescription, non-prescription medications, herbal products, vitamin and supplements) you are taking.

    It is not recommended to use efavirenz on pregnant patients. Discuss with your doctor the risks and benefits involved prior to starting your treatment.

    Mothers infected with HIV are discouraged from breastfeeding due to the risk of transmitting the virus to the baby.

    Side Effects of taking Efavirenz?

    Your doctor prescribed this medicine after carefully analyzing the benefits over the risks involved. Most people do not experience any serious side effects. Common side effects which includes signs of :

  • trouble sleeping
  • drowsiness
  • dizziness
  • abnormal dreaming
  • difficulty concentrating
  • diarrhea
  • headache
  • The above mentionedse side effects may start in the first 1 or 2 days of treatment and usually disappears after the first 2 to 4 weeks.

    Inform your doctor immediately if you experience any of the serious side effects mentioned below.

  • any signs of skin rash.
  • signs of infection (swollen lymph nodes, fever, cough, chills, trouble breathing, non-healing skin sores)
  • signs of liver problems (yellowing of skin or whites of the eye, vomiting, pale stools, loss of appetite, weight loss, dark urine, nausea, diarrhea)
  • signs of pancreatitis (fever, chills, nausea, abdominal pain on the upper left side, back pain, rapid heartbeat, swollen abdomen)
  • vision problems (balance or eye movement)
  • trouble speaking or swallowing
  • increased fat on the upper back, breasts, neck, and around the trunk and loss of fat from the legs, face and arms. Redistribution, accumulation or loss of body fat may occur in patients receiving combination antiretroviral therapy. Tell your doctor if you notice changes in your body fat.
  • bone problems. Some patients taking combination antiretroviral therapy may develop a bone disease called osteonecrosis (death of bone tissue caused by loss of blood supply to the bone). The length of combination antiretroviral therapy, corticosteroid use, alcohol consumption, severe immunosuppression, higher body mass index, among others, may be some of the many risk factors for developing this disease. Signs of osteonecrosis are joint stiffness, aches and pains (especially of the hip, knee and shoulder) and difficulty in movement. If you notice any of these symptoms please inform your doctor.
  • In case you show the following symptoms get to the emergency clinic.

  • Signs of a severe skin reaction (peeling, blistering, a rash that spreads quickly, a rash covering a large area of the body, a rash combines with fever or discomfort

    Efavirenz may make you dizzy. Consumption of alcohol or marijuana can increase the risk of drowsiness. Do not operate machinery, drive a car or do anything that require alertness until you can do it safely. Understand how this medication affects you. Do not take any alcoholic beverages.

  • anti-cancer medications (docetaxel, vincristine, cabazitaxel, doxorubicin, etoposide, ifosfamide)
  • alpha blockers (tamsulosin, silodosin, alfuzosin, doxazosin)
  • mifepristone
  • mitotane
  • modafinil
  • aripiprazole
  • antipsychotics (chlorpromazine, clozapine, haloperidol, olanzapine, quetiapine, risperidone)
  • narcotic pain relievers (codeine, morphine, oxycodone, fentanyl)
  • muscle relaxants
  • IMPORTANT: Inform the doctor, if you are currently taking any of the following medicines:

  • astemizole or terfenadine (used to treat allergy symptoms)
  • bepridil (used to treat heart disease)
  • cisapride (used to treat heartburn)
  • ergot alkaloids (for example, ergotamine, dihydroergotamine, ergonovine, and methylergonovine) (used to treat migraine and cluster headaches)
  • midazolam or triazolam (used to help you sleep)
  • pimozide (used to treat certain mental conditions)
  • St. John's wort (Hypericum perforatum) (a herbal remedy used for depression and anxiety)
  • If you are taking any of these medicines, tell your doctor immediately. Taking these medicines with Efavirenz could create the potential for serious and/or life-threatening side-effects or stop Efavirenz from working properly.
