Cilanem 1gm Injection is a combination of antibiotic medicines which consist of Imipenem 1000mg and Cilastatin 1000mg. It fights against the infection-causing microorganisms to prevent further infections. Usually prescribed for pneumonia, sepsis, endocarditis, joint infections, intra-abdominal infections, and urinary tract infections. Dosage is usally injected into a vein or muscle.

This medication is a broad spectrum beta-lactam antibiotic, prescribed for severe body infections.

Cilanem 1gm Injection is typically prescribed for the treatment of serious lower respiratory, intra-abdominal, urinary tract, gynecologic, bone, joint, endocarditis, bacterial septicemia, skin and skin structure infections and polymicrobic infections.

Dosage and direction of taking Cilanem 1gm Injection

Cilanem 1gm Injection should only be administered under the supervision of your doctor or nurse.

It is given by a drip into a vein (intravenous infusion) over 20 to 60 minutes.

Do not miss any pre-scheduled appointments for receiving the Injection. The course of the medicine should be completed even if you feel better.

IMPORTANT: DO NOT self administer this medicine.

The dosage is based on your weight, medical condition, and response to treatment including other medications you may be taking. Be sure to tell your doctor and pharmacist about all the products you use (including prescription drugs, nonprescription drugs, and herbal products).

Avoid drinking alcohol while taking Cilanem 1gm Injection. Alcohol can cause excessive drowsiness with this medicine.

How Cilanem 1gm Injection work

Cilanem 1gm Injection contains equal quantities of imipenem and cilastatin [1000mg/1000mg]. Imipenem and cilastatin has the ability to kill a wide variety of bacteria. Imipenem is an active antibiotic agent which works not allowing bacteria to form cell walls, thereby making the bacteria break up and die.

Imipenem is a broad-spectrum betalactam antibiotic used for severe bacterial infections caused by susceptible organisms. However, Imipenem is easily broken down by renal enzyme dehydropeptidase when injected alone which may cause kidney damage aswell, thus making it less effective. Due to this it is given in combination with cilastatin, a DHP-I inhibitor which increases half-life and tissue penetration ability of imipenem. Imipenem with cilastatin, like other carbapenems, binds to bacterial penicillin-binding proteins and interferes with bacterial cell wall integrity and synthesis. It has activity against many aerobic and anaerobic Gram-positive and Gram-negative organisms, including Staphylococcus aureus, Streptococcus pyogenes, S. agalactiae, S. viridans- group streptococci, Enterococcus faecalis, Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis, Bacteroides fragilis and Peptostreptococcus species.

Precautions while taking Cilanem 1gm Injection

Tell your doctor if you are

  • allergic to imipenem, cilastatin, or any other part of this drug.
  • allergic to penicillin as this medicine should be avoided or used with caution in penicillin allergy.
  • allergic to any medicine. Make sure to tell about the allergy and what signs you had.
  • having any rash; hives; itching; shortness of breath; wheezing; cough; swelling of face, lips, tongue, or throat; or any other signs
  • IMPORTANT: Further tell the doctor, if you:

  • have or had any kidney disease or liver disease.
  • have brain disorders; such as histroy of seizures, head injury, or tumor
  • have CNS disorders
  • Stomach or intestinal diseases such as colitis
  • have meningitis
  • have renal or hepatic impairment

  • Alcohol can cause excessive drowsiness with this medicine.

    This medicine should be used during pregnancy only if the potential benefit justifies the potential risk to the mother and baby.

    If you are pregnant already, or breastfeeding think you may be pregnant, or are planning to have a baby, ask your doctor for advice before taking this medicine

    This medicine is not recommended for neonates less than 3 months; The doctor will give the right dose depends on your child's age and disease

    About other prescription and nonprescription medicines, vitamins, nutritional supplements, and herbal products you are taking or plan to take. Cilanem may affect the way other medicines or products work, and other medicines or products may affect how Cilanem works.

    Side Effects of taking Cilanem 1gm Injection

    Like all medicines, this medicine can cause side effects, although not everybody gets them. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

    Common side effects for both forms are:

  • Upset stomach
  • Vomiting
  • Stomach pain
  • Major side effects requiring medical attention:

  • Diarrhea
  • Rash
  • Fever
  • Facial swelling
  • Difficulty breathing

  • Hallucination
  • Fungal infection due to yeast
  • Bilirubin level increased
  • Low blood pressure
  • Increased blood platelets
  • Creatinine

    Drug interaction with Cilanem 1gm Injection

    Avoid taking alcohol while you are taking Cilanem injection.

    IMPORTANT: Inform the doctor, if you are currently taking any of the following medicines:

  • ganciclovir which increased risk of seizures
  • Valproic acid or sodium valproate (used to treat epilepsy)
  • Probenecid (used to treat gout)

  • If you are taking any of these medicines, tell your doctor immediately. Taking these medicines with Cilanem could create the potential for serious and/or life-threatening side-effects or stop Cilanem from working properly.

    Cilanem injection should not be used if you have the following conditions

  • kidney, urinary and liver problems
  • gastrointestinal diseases
  • hypersensitivity to Cilnem or to any of its active ingredients
  • inflammation in the large intestines
  • convulsions
  • Storage of Cilanem 1gm Injection

    Keep this medicine out of the sight and reach of children.
    Store at 15°C - 30°C (59°F - 77°F); protect drug from excessive light, humidity, and temperature over 30°C (86°F)
    Keep Cilanem 1gm Injection in the container till you handover to your health provider.
    Throw away any Injection that are no longer needed or expired (out of date).
    Medicines should not be disposed of via waste water or household waste. Ask your pharmacist how to dispose of medicines no longer required. This will help to protect the environment.
    Cilanem 1gm Injection should not be used after the given expiration date.
